Woodland Care Center LLC is conveniently located in the mid San Fernando Valley area of Southern California. We specialize in professional and individually structured rehabilitation and nursing for residents who have lost functional abilities due to illness or injury, with a goal to return them to their prior functional level and living arrangement. We also offer nursing care for those seeking long term care. We take most major insurance plans.
Short-term Services -
If you are seeking short-term medical and/or rehabilitative services, we will focus our efforts working closely with you and your family on achieving the most successful functional outcomes so that you can return home as quickly as possible. A dedicated team of healthcare professionals will develop an individualized care plan that is designed to address your specific needs. Supporting the nursing team is a group of licensed therapists who provide physical, occupational and speech therapies for patients recovering from major surgery, stroke, neurological and orthopedic conditions and other illnesses, injuries and disabilities. We have dedicated a special unit of 50 beds designed exclusively for patients who have as their primary goal to return home as quickly and as safely as possible. We are guided by the principle that quality healthcare is best achieved when we work together.
Long-term Care Services -
Our long-term care units are designed for individuals who need 24-hour nursing care and may be chronically ill, frail or experiencing a very slow recovery from an illness or injury. These units offer private and semi-private room accommodations including room furnishings, comfortable common areas and patios . In addition, we offer respite or vacation care that allows families an opportunity to take a brief break from the demands of caring for a loved one round-the-clock.
We accept Medicare, Medicaid, managed care insurance and private pay.
